ABSTRACT

Objective:To investigate the clinical and imaging features of basal ganglia germinoma in children and adolescents, so as to improve the accuracy of early diagnosis.Methods:The clinical symptoms and imaging findings of 10 cases of basal ganglia germinoma diagnosed by surgical examination or diagnostic radiotherapy in Xinhua Hospital Affiliated to Medical College of Shanghai Jiaotong University from January 2010 to December 2019 were analyzed retrospectively.Results:There were 9 males and 1 female, aged from 5 to 15 years, with an average age of 9.7 years.There were 10 cases of limb weakness or mild hemiplegia, 1 case of headache and vomiting, and 1 case of extrapyramidal tract sign. There were 9 cases of computed tomography (CT) examination, 5 cases of high density, 4 cases of mixed density, and 3 cases of intrafocal calcification. 10 cases of routine magnetic resonance imaging (MRI) examination, MR T1-weighted imaging (T1WI): 5 cases of low signal, 3 cases of slightly low signal, 1 case of equal low signal, 1 case of equal slightly low signal; MR T2-weighted imaging (T2WI): 3 cases of high signal, 5 cases of slightly high signal, 2 cases of high and low mixed signal; T2 fluid attenuated inversion recovery (T2FLAIR): 1 case of low signal, 2 cases of slightly high signal, 1 case of iso-high signal, 5 cases of high signal, 1 case of high and low mixed signal; Diffusion weighted imaging (DWI): 2 cases of low signal, 1 case of equal signal, 1 case of equal slightly high signal, 4 cases of slightly high signal, 2 cases of high signal; 1 case of functional imaging susceptibility-weighted imaging (SWI): 1 case of low signal; 10 cases of MRI enhancement: 2 cases of no enhancement, 5 cases of mild enhancement, 3 cases of moderate/obvious enhancement, and the enhancement methods can be seen in spot, patch, mass and ring enhancement; Functional imaging magnetic resonance angiography (MRA) in 2 cases: 1 case had slender middle cerebral artery, fewer branch arteries than the opposite side, and 1 case showed no obvious abnormality; 2 cases of positron emission (PET)-CT: the metabolism of methionine (MET) on the affected side was significantly increased in 2 cases.Conclusions:Germinoma in the basal ganglia of children and adolescents are more common in men, with weakness or mild hemiplegia as the main symptom. The imaging findings have certain characteristics. Imaging findings combined with relevant clinical data can improve the accuracy of early diagnosis of germinoma in the basal ganglia.

ABSTRACT

Peri-implantitis, characterized by inflammation of tissues around implants and gradual loss of supporting bone tissue, has become one of the main causes for implant failure. Thoroughly removing the plaque biofilm on the implant surface is the first principle in the treatment of peri-implantitis. For this reason, various decontamination methods have been proposed, which can be divided into 2 categories: Removing biofilm and killing microorganisms according to the effect of plaque biofilm on the implant surface. However, at present, there is no decontamination method that can completely remove the plaque biofilm on the implant surface, and it lacks of clinical recommended guidelines. To understand the advantages and disadvantages, effectiveness and safety for different implant surface decontamination methods is of great significance to guide the clinical selection for peri-implantitis treatment.

ABSTRACT

Pleural infection is a common disease associated with increasing morbidity and high mortality. Factors such as age, geographical area, settings of infection (hospital or community acquired)or certain underlying diseases can influence the microbiology of pleural infection. Thus, understanding the microbiology of pleural infection is of vital importance to guide therapeutic decisions. Since the positive rate of conventional cultures is low among cases of pleural infection, new diagnostic methods such as 16S ribosomal RNA (16S rRNA)and metagenomic next-generation sequencing(mNGS)have broad prospects in clinical applications. 16S rRNA and mNGS improves the etiologic diagnosis of pleural infection, facilitating the decision of appropriate anti-infection regimens. The treatment of pleural infection includes appropriate anti-infection regimen, pleural drainage, intrapleural therapy and the need for medical thoracoscopy or surgery. It is convinced that the use of intra-pleural DNase and t-PA has exhibited excellent therapeutic effects in pleural infection and reduced the need for surgery. This paper summarizes recent literatures on the microbiologic characteristics of pleural infection and progress of diagnosis and treatment, aimed to provide clinical evidence for the management of pleural infection.

ABSTRACT

Objective To investigate the relationship between elderly hypertension patients with frailty syndrome and oral medication compliance. Methods Two hundred elderly patients with hypertensionwho visited the department of cardiology of the First Hospital Affiliated to Dalian Medical University from March 2015 to March 2017were randomly selected. General information was recorded for each patient. The Tilburg Frailty Indicator (TFI), Healthy behavior inventory (HBI) and medication adherence inventory (mmas-8) were assessed for each patient. The population variable and TFI were taken as independent variables, and the mmas-8 score and HBI score were taken as dependent variables to evaluate the linear correlation between each independent variable and each dependent variable. The above independent variables were used as covariates and the results were corrected by disordered multiple Logistic regression analysis. Results The average age of 200 patients was (73.5 ± 7.5) years old, including 103 women (51.5%, 103/200), 97 males (48.5%, 97/200), and the average medical history of hypertension was (12.5 ± 6.1) years. There were 74 patients (37%, 74/200) and 126 patients (63%, 126/200). The mean TFI was 4.3 ± 3.0. The average scores of physical, psychological and social factors were 7.6±3.1, 3.9±2.0, 1.4±0.9, and the average HBI was 3.3±0.3. Among them, the average scores of healthy eating habits, preventive health care, positive mentality and healthy behavior were 3.3±0.8, 3.3±0.4, 3.4± 0.5, and 3.4 ± 0.5, respectively. The mean MMAS-8 was 5.2 ± 2.0, with 144 patients (72%, 144/200) with low compliance and 38 (19%, 38/200) with moderate compliance. Age (Rho:- 0.443), history of hypertension (r:-0.421), TFI score (Rho:-0.483) were negatively correlated with drug adherence (P<0.05). Married/cohabiting patients (tau_b: 0.293), education level (tau_b:0.391), monthly income (tau_b: 0.398) were positively correlated with drug compliance, and the latter two were significantly positively correlated with drug compliance (P<0.01). Monthly income (tau_b:0.371) was significantly positively correlated with HBI, and TFI score (Rho:-0.364) were significantly negatively correlated with HBI, P<0.01. Logistic regression analysis suggested that education degree was a positive determinant of drug compliance (mmas-8) (B=0.81,P=0.01), and monthly income was a positive determinant of health behavior (HBI). Frailty syndrome (TFI) is a negative determinant of poor drug adherence (B=-0.11, P<0.01) and poor health behavior (B=-0.71, P=0.03). Conclusions Frailty syndrome is an important independent factor in predicting drug adherence and adherence to healthy behaviors. High education level improves patient compliance, and good financial status can make patients more active in adherence to healthy behaviors.

ABSTRACT

The study included 197 thyroid nodules which were confirmed by fine-needle aspiration cytology or histopathologic examination. All nodules were graded with malignancy risk stratification of thyroids nodule accordingto the 2015 American Thyroid Association(ATA)management guidelines. Both color Doppler flow imaging (CDFI) and superb microvascular imaging(SMI) were used to classify blood flows of thyroid nodules according to Adler's grading criteria. Morphologic and distribution features of blood flow were also observed by monochrome (mSMI). The optimal threshold drawing from ROC curve and diagnostic efficacy of single and combinative modality were calculated. The results showed that mSMI was more sensitive to detect blood flow of thyroid nodules than other Doppler techniques(P<0.01). Microvascular morphologic features between benign and malignant thyroid nodules were significantly different(P<0.01). The area under ROC curves of ATA,mSMI,and ATA+mSMI were 0.745, 0.740,and 0.834,respectively,suggesting that the diagnostic performance of ATA+mSMI is superior to that of ATA or mSMI alone. There was no significant deference in the sensitivity among ATA, mSMI, and ATA+mSMI (P>0.05). But the specficity and accuracy of combinative modality ATA+mSMI was significantly higher than that of ATA or mSMI alone(P<0.05).

ABSTRACT

Objective To investigate the characteristics of falls as an adverse event in the hospitalization of elderly patients taking sedatives and hypnotics drugs.Methods Data of elderly patients treated in Department of General Internal Medicine,Beijing Chaoyang Hospital from July 2011 to July 2015 were collected.All patients were divided into medicine-taking group and non-medicinetaking group according to the history of taking sedatives and hypnotics during the hospital stay.Different characteristics of fall as an adverse event were compared between the two groups,and fall related nursing interventions were presented.Results A total of 3 834 patients were collected for analysis in this study.Of these patients,the fall as an adverse event occurred in 12 cases,the total incidence was 3.1%,in whom 8/638 (12.5%) in medicine-taking group and 4/3196 (1.3%) in nonmedicine-taking group(x2=21.72,P<0.01).Of 8 cases with the fall as an adverse event in medicinetaking group,the fall occurred in 6 cases(75%)during 24 ∶ 00-6 ∶ 00.However,of 4 cases with the falls in non-medicine-taking group,the fall in 3 cases(75 %)occurred during 6 ∶ 00-18 ∶ 00.In medicinetaking group,the fall occurred beside their beds in 62.5 % (5 cases).And in 37.5 % (3 cases)did in their bathrooms.In non-medicine-taking group,the fall occurred in 2 cases (50.0%) beside their beds,in 1 case did in the bathroom,1 case did in the passage.Multiple Logistic regression analysis showed that age > 75 years(OR=1.26,95 %CI:1.07-1.48),elevated nutritional risk score(NRS-2002 ≥3 scores) (OR=10.92,95%CI:1.79-66.46)and elevated risk for fall or out of bed(risk score ≥18 scores) (OR =19.08,95% CI:4.02-90.47)were the independent risk factors for falls as an adverse event among medicine-taking group.Conclusions A use of sedatives and hypnotics is related with the increase of fall as an adverse event in elderly patients during a hospital stay.With the aim of preventing or reducing the incidence of fall as an adverse event,nursing interventions should be carried out based on the characteristics of fall as an adverse event in elderly patients taking sedatives and hypnotics.

Objective:To explore leuncine-rich repeats and immunoglobulin-like domains protein 1 (LRIG1) expression in oral verrucous carcinoma(OVC) and its possible mechanism of tumor suppression.Methods:Paraffin specimens of OVC (n =15) and oral squamous cell carcinoma(OSCC,n =30) and corresponding adjacent normal tissues,and 15 normal mucosa tissues(NM) as control from injured patients were collected.The expression levels of LRIG1 and Bcl-2 were examined using immunohistochemical SP method.The correlation between the expression levels of LRIG1 and Bcl-2 was determined using two tailed Pearson's correlation.Results:LRIG1 was significantly lower in OVC tissue than that in NM tissue,but higher than in OSCC tissue.The expression levels of Bcl-2 in OVC were significantly higher than that in NM,but lower than in OSCC.The expression level of LRIG1 was negatively correlated with expression level of Bcl-2 in NM,OVC and OSCC tissue.Conclusion:LRIG1 may inhibit the expression of Bcl-2 and the development of OVC.

Geiwang-9 is a traditional prescription which is a frequently used powder in traditional Mongolian medicine.It has been adopted in the treatment of liver diseases with remarkable therapeutic effects.In recent years,many studies have made progress with quality control,pharmacology,pharmacological effects and clinical application of Geiwang-9.This paper reviewed the research progress on clinical applications and experimental studies of Geiwang-9 based on literatures on Geiwang-9 in recent 20 years.It provided a beneficial reference for further research and broadened the clinical application of Geiwang-9.

ABSTRACT

AIM: Ischemic post-conditioning ( PC) plays an important role in cardioprotection from ischemia /reperfusion ( I/R) injury in the young heart but not in the aging hearts .The physiological and pathological roles of hydrogen sulfide ( H2 S) in the regulation of cardio-vascular functions have been recognized .Whether H2 S is involved in the recovery of PC-induced cardioprotection in the aging hearts is unclear.METHOD:The male Wistar young rats (3-month-old), the aging rats (24-month-old), primary cultured cardiomyocytes and the aging cardiomyocytes induced by D-galactose suffered from I/R (or H/R) and PC.RESULTS:I/R (or H/R) decreased H2S production rate and cystathionine γ-lyase (CSE) expression, aggravated cardiomyocyte damage , apoptosis, myocardial infarct size and oxidative stress, reduced cardiac function, increased the levels of Bcl-2, cleaved caspase-3 and caspase-9 mRNA and proteins, promo-ted the release of cytochrome c and mPTP opening, down-regulated the phosphorylation of ERK 1/2, PI3K, Akt and GSK-3βand mito-chondrial membrane potential , up-regulated the phosphorylation of IκBα, NF-κB, JNK2 and STAT3, and inhibited PKC-ε transloca-tion and mitochondrial ATP-sensitive K channels (mitoKATP) in isolated young and aging hearts as well as normal and aging cardiomyo-cytes.PC suppressed myocardial I/R injury in the young heart but not in the aging hearts .Supply of NaHS not only increased PC-in-duced cardioprotection in the young hearts and cardiomyocytes , but also attenuated I/R injury and significantly recovered the cardiopro-tective role of PC in the aging hearts and cardiomyocytes .CONCLUSION:The exogenous H 2 S restores PC-induced cardioprotection through inhibiting oxidative stress via the down-regulation of NF-κB and JAK2-STAT3 pathways and mPTP opening by the up-regulation of ERK1/2-GSK-3β, PI3K-Akt-GSK-3βand PKC-ε-mitoKATP pathways in the aging hearts and cardiomyocytes .These findings provide a novel potential target for the treatment of aging ischemic cardiomyopathy .

ABSTRACT

AIM:To investigate the relationship between polyamine metabolism and hypoxia /ischemia ( H/I)-induced cell apoptosis and to determine the mechanisms by which exogenous spermine protects cell apoptosis against AMI in rats .METHOD:The left anterior de-scending coronary artery ( LAD) of the Wistar rats were ligated , and neonatal rat cardiomyocytes were placed under hypoxic conditions for 24 h to establish the model of AMI (or H/I).Exogenous spermine was administered by intraperitoneal injection (2.5 mg/kg daily for 7 days) in vitro and subjected to the cell medium at 5μmol/L as a pre-treatment therapy.RESULTS:AMI (or H/I) induced an increase in polyamine catabolized enzyme SSAT and a decrease in polyamine biosynthesis enzyme ODC , which result in endogenous spermine and spermidine decrease and putrescine increase .At the same time, AMI ( or H/I) lowered cardiac function , increased cTnI and CK-MB concentrations , aggravated myocardial infarct size , cardiomyocyte damage and apoptosis , raised ROS generation , increased the expression of cleaved caspase-3, cleaved caspase-9 and endoplasmic reticulum stress (ERS)-related proteins, promoted the release of cytochrome C and mPTP opening , down-regulated Bcl-2 expression and the phosphorylation of ERK 1/2, PI3K, Akt and GSK-3β, and activated PERK and eIF 2αphosphorylation .Spermine pre-treatment reversed the above-motioned changes .CONCLUSION:AMI ( or H/I ) could induce cardiomyocyte apoptosis and polyamine metabolism disorder .Exogenous spermine attenuates cardiac injury through scavenging the ROS and inhibiting mPTP opening and ERS injury .These findings provide a novel target for the prevention of apoptosis in the setting of AMI .

ABSTRACT

ObjectiveTo explore the relationship between serum sCD40L level and coronary stenosis degree in acute coronary syndromes(ACS) patients with different glucose tolerance conditions.Methods154 patients with ACS were divided into normal glucose tolerance group ( NGT),impaired glucose tolerance group (IGT) and type 2 diabetes group (T2DM) according to the OGTT.The levels of serum sCD40L were detected by ELISA,and all patients underwent coronary angiography in order to calculate their Gensini score.Then the relationship between serum sCD40L level and coronary stenosis was analyzed by statistics.ResultsThe Gensini scores of IGT[(41.8 ± 19.8)score] and T2DM groups [(40.1 ± 18.7)score] were higher than NGT group [(21.9 ± 15.3)score] ( P <0.01 ),but there was no statistical difference between IGT and T2DM ( P > 0.05 ) group.The serum sCD40L levels of IGT[(2.46 ± 0.79 )ng/ml]were higher than NGT group [(2.12 ± 0.81 )ng/ml] ( P < 0.05 ),and the serum sCD40L levels in T2DM group [( 2.57 ± 0.68) ng/ml] were obviously higher than NGT group [( 2.12 ± 0.81 ) ng/ml] ( P < 0.01 ).The serum sCD40L levels in T2DM group were higher IGT group,but there was no statistical difference between T2DM and IGT groups( P >0.05).Statistical analysis revealed that serum sCD40L levels had no statistical difference among the three groups and it had no relationship with coronary stenosis ( r =-0.147,P >0.05).ConclusionsCD40/CD40L activation would occur in the patients of ACS with wildly abnormal blood sugar,not only in those who had diabetes with much higher blood sugar.The serum sCD40L level in the ACS patients with abnormal glucose tolerance had no relationship with the severity of coronary stenosis.

ABSTRACT

A molecularly imprinted polymer (MIP) using benzoic acid as template molecule, 4-vinyl pyridine (4-VP) as functional monomer, ethylene dimethacrylate (EDMA) as cross-linker, was prepared by bulk polymerization. The needle-type gas concentrator was developed using the MIP as adsorption medium. The device was coupled with gas chromatography (GC) for the analysis of volatile organic compounds (VOCs). The effect of polymerization conditions on adsorption property, such as polymerization time, ratio of the reagents, pre-polymerization time, type of solvents, type of template molecules, has been evaluated. The results of gas chromatographic analysis demonstrated that the optimum conditions for getting the best adsorption performance of the synthesized were polymerization time 6 h at 60 ℃, the ratio of the reagents (template molecule : functional monomer : cross-linker) 1∶ 4∶ 20, pre-polymerization time of 3 h, acetonitrile as solvent, benzoic acid as template.
